Inconsistent availability of insulin, glucometers, and test strips at PHC and CHC levels compels families to seek expensive private care.
Frontline healthcare workers lack adequate training and clearly defined responsibilities for pediatric insulin management, counselling, documentation, and follow-up.
Absence of a district-level registry and monitoring system results in missed cases, weak continuity of care, and delayed management of complications.
Existing primary healthcare infrastructure offers an opportunity to integrate comprehensive pediatric Type 1 Diabetes care without creating parallel systems.

Fifty-four percent of enrolled children showed improvement in Hb1Ac levels within eight months of implementation.
Twenty-six point six percent of children successfully reduced their daily insulin dosage.
Hospital admissions due to diabetic ketoacidosis reduced by sixty-six percent compared to earlier records.
Forty-three out of one hundred twenty-four underweight children gained body weight within six months.
Forty-eight percent of children transitioned to government healthcare facilities for ongoing diabetes management.
